Registration Renewals Yearly Update Form

    Note: This information is required even if an individual’s compensation is not required to be reported on The Foundation’s Form 990. In other words, we need this information for the top 5 highest compensated individuals regardless of the amount of compensation. If another organization partially pays an employee’s compensation, still provide us with the compensation amount, but indicate the paying organization.

    Furthermore, if any of the information that you provide in response to this form changes throughout the year, please let us know. For example, Board member elections or resignations, hiring of principal salaried executives, or entering into professional fundraising contracts. Renewals are prepared using the most recently provided information from the Foundation.

    This form, upon completion, together with the Form 990 and audited financial statements, should provide us with the necessary information to complete annual renewals; however, we may need to request additional information throughout the year due to periodic changes to state forms or as a follow-up to information provided on the Form 990.
